## 1. Choose this provider when

CDC PLACES (Population Level Analysis and Community Estimates) on data.cdc.gov: model-based estimates of the share of adults with 40 measures, from chronic diseases (diabetes, obesity, high blood pressure, high cholesterol, COPD, asthma, coronary heart disease, stroke, depression, arthritis, cancer) to risk behaviors (smoking, binge drinking, physical inactivity, short sleep), prevention (health insurance, checkups, screenings), health status, disabilities and social needs, for every county, place, census tract and ZIP Code Tabulation Area in the 50 states and DC. Releases from 2020 on; the latest uses BRFSS survey data from 2022 and 2023. Each estimate carries 95% confidence limits and total and adult population, with a derived count of adults and a summary over every matching location, the denominators patient-based epidemiology and drug forecasting start from. Public domain U.S. Government data; every result carries CDC's attribution and terms.

## 2. Minimal request

Call `POST https://api.firecrawl.dev/v2/scrape` with `{ alexandria: { provider, capability, options } }`. For a batch, send `{ alexandria: [...] }` with up to 10 calls.

```json
{
  "provider": "data-cdc-gov",
  "capability": "prevalence/search",
  "options": {
    "level": "county",
    "locations": [
      "48201",
      "48113",
      "48439",
      "48029",
      "48453"
    ],
    "measure": "DIABETES",
    "state": "TX"
  }
}
```

## 3. Add provider options

Use only the options needed for the task:

- `county_fips` (string): 5-digit county FIPS code (`48201`): the census tracts of that county. Tract level only. Pattern: ^[0-9]{5}$. Example: `48201`
- `level` (string): Geography: `county` (default), `place` (cities, towns and census designated places), `tract` (census tracts; needs `state`, `county_fips` or `locations`) or `zcta` (ZIP Code Tabulation Areas; no state, so select them by `locations` or page through all). Example: `county`
- `limit` (number): Locations per page. `total_count` and `summary` cover every match. Example: `100`
- `locations` (string[]): 1 to 100 location codes at the chosen level: 5-digit county FIPS (`48201`), 7-digit place FIPS (`1714000`), 11-digit tract FIPS (`48201100001`) or 5-digit ZCTA (`77002`). County, place and tract codes must start with a state's FIPS code (and `state`'s, when given). Codes without an estimate are left out with a warning; when none of them exists for any measure, the call is not_found. Example: `["48201"]`
- `measure` (string, required): PLACES measure id. Health outcomes: ARTHRITIS, BPHIGH (high blood pressure), CANCER (cancer other than skin, or melanoma), CASTHMA (current asthma), CHD (coronary heart disease), COPD, DEPRESSION, DIABETES (diagnosed), HIGHCHOL (high cholesterol among adults ever screened), OBESITY, STROKE, TEETHLOST (all teeth lost, adults 65 and older). Prevention: ACCESS2 (no health insurance, ages 18-64), BPMED (taking blood pressure medicine, among adults with high blood pressure), CHECKUP, CHOLSCREEN, COLON_SCREEN (ages 45-75), DENTAL, MAMMOUSE (women 50-74). Risk behaviors: BINGE, CSMOKING, LPA (no leisure-time physical activity), SLEEP (short sleep). Health status: GHLTH (fair or poor health), MHLTH (frequent mental distress), PHLTH (frequent physical distress). Disabilities (2023 release on): DISABILITY (any), COGNITION, HEARING, INDEPLIVE, MOBILITY, SELFCARE, VISION. Social needs (2024 release on): EMOTIONSPT, FOODINSECU, FOODSTAMP, HOUSINSECU, LACKTRPT, SHUTUTILITY; LONELINESS (2025 on); ISOLATION (2024 only). Releases up to 2023 only: CERVICAL, COREM, COREW, KIDNEY (chronic kidney disease). A measure the chosen release lacks is invalid_input naming the release's measures. Example: `ACCESS2`
- `name` (string): County or place name, matched case-insensitively and exactly as CDC writes it (`Harris`, `Chicago`); for counties a trailing County, Parish or Borough is optional (`Harris County`). County and place level only, with `state`. A name that matches no location in the release is not_found. Example: `<name>`
- `offset` (number): Matches to skip: `next_offset` of the previous page. Example: `0`
- `release` (number): PLACES release year. Default: the latest release at the chosen level. Releases before 2024 have no adult population; the 2020 county release lacks location codes and is refused. Example: `2020`
- `sort` (string): `value_desc` (default, highest first), `value_asc` or `location` (by location code). Suppressed values sort last; ties go by location code. Example: `value_desc`
- `state` (string): USPS code of one of the 50 states or DC (`TX`). Not at zcta level, where CDC gives no state. Needed with `name`, and at tract level unless `county_fips` or `locations` is given. Pattern: ^[A-Za-z]{2}$. Example: `<state>`
- `value_type` (string): `crude` (default): the share of the location's adults. `age_adjusted`: standardized to the 2000 US population so locations with different age structures compare; county and place level only, and estimated adults are not derived from it. Example: `crude`

## API reference-derived contract

The following capability contract is generated from the same normalized Alexandria API reference exposed in the API spec.

### Search

- Capability: `prevalence/search`
- Description: CDC PLACES model-based estimates of adult prevalence (percent of adults aged 18 and older) for one of 40 measures: chronic diseases (diabetes, obesity, high blood pressure, high cholesterol, COPD, asthma, coronary heart disease, stroke, depression, arthritis, cancer), risk behaviors, prevention and screening, health status, disabilities and social needs, by county, place (city or town), census tract or ZCTA across the 50 states and DC, from the latest PLACES release or an older one (2020 on). Filter by state, location codes, county or place name, or the tracts of one county; sort by value or location; page with limit and offset. Each location has its crude prevalence (or, at county and place level, age-adjusted) with 95% confidence limits, total and adult population, an estimated count of adults with the measure (derived: crude prevalence times adults), coordinates and data year; suppressed estimates are kept with a null value and CDC's footnote. A summary over every match, not just the page, gives location and suppressed counts, total and adult population, estimated adults and the adult-weighted prevalence (both derived). A measure the release lacks is invalid_input naming the release's measures; named locations that exist for no measure are not_found; an area without estimates for the measure (Kentucky and Pennsylvania lack most 2025 county measures) is an empty result with a `no_estimates` warning.
- Instructions: For adult prevalence of a chronic condition, risk behavior, screening or social need in US counties, cities, census tracts or ZIP code areas, and the number of adults it implies: "adult diabetes prevalence by county in Texas", "obesity in the census tracts of Harris County", "how many adults in Cook County have COPD", "age-adjusted high blood pressure in Chicago". Estimates are modeled from CDC's BRFSS survey and are percent of adults, not case counts. For cancer incidence or mortality by site, use uscs-cdc-gov cancer-statistics/search (PLACES CANCER is self-reported ever-diagnosed cancer with no site). For rare disease prevalence, use orphadata-com rare-diseases/epidemiology. For population by age, sex or income beyond PLACES' totals, use census-gov census/data. For any other data.cdc.gov dataset, use socrata-com open-data/query_dataset with domain data.cdc.gov.
- Cost: 5 credits per call
- Capability file: [Search](https://firecrawl.dev/alexandria/agents/providers/data-cdc-gov/prevalence/search)
